This survey is about using de identified data. This means that the researcher or analyst does not know your name or the details which identify who you are.

When health and care organisations are using de-identified data to plan services and undertake research, please tell us which of the following are important to you:

Make sure that people and communities continue to be at the heart of decision making on the use of data.
Be open and transparent around the uses of data – ensuring everyone is clear on the who, what, when, why and how of data use.
Provide accessible information about how we use and look after your data. Including the purpose and benefits to sharing data, the different reasons for using your data and how the different uses may impact you. For example, data sharing for: Direct care, Research, Planning.
Strong data security practices in place, and an open and honest dialogue between you and our health and care services. Providing robust responses and clarity on any process changes.
Provide you with clear information around the use of data so that you can make informed decisions. 
We will promote strong and robust security practices around the use of data, adopting a consistent approach.  We will act in good faith and be honest and open where mistakes have been made, ensuring that lessons are learnt and implemented.
